Sacraments Certificate Request

If you need a certificate for a Baptism, Communion, Confirmation, or Marriage from St. Helen Church, St. Aloysius Church, Holy Rosary or St. Mark Church, please follow the steps below:


  • Payment: First, pay the $15 fee certificate fee-includes 2 copies
  • Form Submission: After payment, fill out the Sacraments Certificate Request Form with the necessary details.


Once your form is submitted, our parish staff will contact you to arrange the pickup and receipt of your payment. 


Certificates will be ready for pick up within 7-10 Business days at our St. Helen Church Office at 2315 W. Augusta Blvd., Chicago, IL 60622.
